I sent an email to this address about a week ago with a minor question about the paperwork...perhaps someone here can answer for me?
On the first two pages, there is a spot for two initials for the chapter at the bottom of the page. Do both lines on both pages need to be initialed? Also, do they have to be initialed by the Administrator and Assistant Administrator who signed on the last page or could my Book Keeper initial, for example?

Both places have to be initialed, and they have to be initialed by the same persons who signed page two of the contract.
That's pretty standard for many complex contracts, the intials are needed to specifically acknowledge that you're aware of and agreeing to the wording in the referenced section. Just wait until you sign a car loan or mortgage agreement - your wrist will get tired.

I sent an email trying to update our chapter information a while ago and have not recieved a response yet. I sent a pm to graymael as well, no response. :-\ do i need to send it again?

If you mean the website info, then no, sending it again will not help. BH has made it clear that this will not be updated. See here for more info:
