Also, with the chain, I do have the foam around the rope, but the cover is completely around the the chain (so it goes rope core, foam rings, then single piece of cloth cover); although it is safe, it's just unconventional.

I'm not an expert flail checker - but I'd expect to see it in this order: rope-cloth-rings. TT the folks in your unit and they can advise you on how to do it right.

There's a printer-friendly version I made floating around here, someplace. I made a LaTeX file of it, so I can just update that whenever and recompile/PDFify it and reupload it with the new changes.
Alric: Send me the definitive versions of the final changes to the rules and I'll update the file, then send it to you to check again.
